How to Create an IT Incident
- Click on "Create a New Information Technology Incident".
- Fill in required fields as defined below.
- Enter your phone number or "None".
- Enter your University or alternate email address.
- Click "Incident Area" and select the appropriate category.
- Provide an explanation of your problem.
- Click "Submit".
Note: After a request is submitted an email will be sent to your University account with a ticket number. Please save this ticket number for future reference.
